Outreach Specialist



PURPOSE OF POSITION

The Outreach Specialist will identify and link members of the public to the Program Specialist for case management and supportive services. The Outreach Specialist will actively engage with communities, building rapport and trust with individuals and groups. They will participate in community events, meetings, and gatherings to understand the needs, concerns, and dynamics of the community they serve.They will assist in developing social media campaigns to raise awareness of mental health issues; risk reduction strategies; substance use, hepatitis, and HIV prevention options, as part of their recruitment strategy.

RESPONSIBILITIES:
 
The major responsibility of this position includes, but are not limited to:

  •  Provide education, benefits navigation  and internal referrals for case management to priority population. 
  • Perform Rapid HIV/DBS/HCV testing.
  • Establish community partnerships and relations. 
  • Create social media marketing campaigns for priority population. 
  • Provide linkages for PrEP, PEP, IART/HIV or STI treatment, and assist with Social Determinants of Health 
  • Participate in a minimum of 3 outreach events per week. 
  • Maintain detailed records of interactions, interventions, and outcomes.
  • Prepare reports to track progress, measure effectiveness, and inform future outreach strategies.
  • Manage day-to-day Drop-in Center/office activities that incorporate social, education, and community building activities.
  • Co-facilitate Evidence-Based Interventions and support groups. 
  • Ensure all collected data and reports are submitted by the prescribed deadline.
  • Must meet at least 85% of program deliverables.
  • Rotating weekends are required. 
  • Other duties and responsibilities can be assigned as necessary. 
EDUCATION
  • High School Diploma or GED + equivalent practical experience required.
  • Associates Degree and equivalent practical experience preferred.
QUALIFICATIONS:
  •  Minimum two (2) year of similar work experience, facilitating groups and performing Outreach/community engagement activities.
  • Commitment and knowledge of issues related to homelessness, mental health, and substance misuse. 
  • Previous work experience with high-risk and HIV+ populations. 
  •  Outstanding writing and communication skills.
  • Certified in HIV/HCV testing or will obtain certification with 60 days of hire. 
  • Proficient with Microsoft Office Suite and related program
  • Previous experience with AIRS database preferred.

Compensation: $42,000.00 - $52,000.00 per year
