POSITION SUMMARY

The MOIA Client Navigator provides direct support, case management assistance, and
navigation services to immigrant community members seeking ESL, legal, benefits, and social
service assistance. The Navigator works closely with HCC Wellness Center, MOIA programs,
and HRI case manager to ensure clients are connected to care and services, navigate program
goals, and meet grant deliverables.

The Navigator conducts assessments, assists with documentation, facilitates referrals, and
ensures participants receive high-quality, culturally responsive support. This position plays a
vital role in promoting access, stability, and equitable service delivery for immigrant families
and individuals. The Client Navigator also collaborates across offices to ensure all program
deliverables are met.

Schedule: Part-time, Monday–Thursday 4:00–8:00 PM, with flexible Fridays to support program
needs. Total 40 hours bi-weekly.

KEY RESPONSIBILITIES

Client Assessment & Navigation

• Conduct client assessments and need screenings to determine eligibility for MOIArelated services.
• Assist clients with documentation gathering, application completion, and paperwork
under the guidance of Legal/Immigration Departments and HRI Case Managers.
• Provide culturally sensitive navigation support and clear explanations of available
services.
• Work closely with HCC Wellness Center and MOIA programs to streamline care
coordination and service access.

Case Management & Follow-Up

• Implement the 6-Step Outreach Process: Recruitment → Intake → Education → Services
→ Data Entry → Follow-Up.
• Perform consistent follow-ups to ensure clients attend appointments, complete
applications, and receive referrals.
• Advocate for clients experiencing barriers to service access and coordinate needed
support.
• Navigate clients through ELS, legal, and HRI program goals and deliverables.

Program Support & Coordination

• Support ESL classes and legal workshops by managing attendance, preparing materials,
and assisting instructors and participants.
• Collaborate with MOIA, ESL, MSA, and YMSM teams for cross-referrals and integrated
service planning.
• Assist with outreach events, community tabling, and partnerships with CBOs, schools,
and community sites.

Resource Navigation


• Connect clients to citywide resources including legal services, ESL programs, benefits
enrollment, and social support programs.
• Maintain working knowledge of available community resources and referral pathways.

Administrative Duties & Reporting


• Maintain accurate, updated client files and submit weekly and monthly tracking logs.
• Complete all required documentation and data entry by program deadlines.
• Assist with flyers, outreach materials, scheduling, and other administrative functions.
• Maintain confidentiality and adhere to organizational policies and service standards.

Professional Standards


• Demonstrate cultural humility, professionalism, and people-first language in all client
interactions.
• Meet at least 85% of program and HRI deliverables.
• Perform other duties as assigned.

EDUCATION & EXPERIENCE

Required:
• Associate Degree in Social Work, Human Services, Education, Public Health, or a related
field
• Minimum 1 year of experience in community engagement, case management, outreach,
or social services

Preferred:
• Bachelor’s degree
• Experience working with immigrant, multilingual, or multicultural communities

Qualifications
• Strong communication and writing skills
• Knowledge of NYC immigrant services, ESL programs, legal supports, and social service 
systems
• Experience in community-based organizations, nonprofit settings, education, or 
health/social services
• Ability to maintain accurate documentation and meet reporting deadlines
• Proficiency with Microsoft Office and digital data systems
• Bilingual or multilingual abilities (Haitian Creole, Spanish, French, etc.) strongly 
preferred
• Cultural humility and commitment to serving diverse populations

Compensation: $25.00 per hour
