Outreach Coordinator
Unit/Department: YMSM
Responsible to: Program Manager

Probationary Period: 3 months
PURPOSE OF POSITION
The YMSM Outreach Coordinator is responsible for planning, implementing, and overseeing outreach and engagement strategies targeting Young Men Who Have Sex with Men (ages 13–29), with a priority focus on Black, Latino, immigrant, and other disproportionately impacted communities.
The Outreach Coordinator leads outreach initiatives, monitors deliverables, and ensures compliance with AIDS Institute program requirements. The role bridges community engagement with care navigation to increase HIV prevention uptake, PrEP initiation, HIV testing linkage, and retention in services. This position requires strong community credibility, cultural responsiveness, data oversight, and deliverable-driven program management.
RESPONSIBILITIES:
The major responsibility of this position includes, but are not limited to:
1. Outreach Strategy & Implementation
· Develop and implement annual and quarterly outreach plans aligned with contract deliverables.
· Identify and maintain outreach sites (community venues, colleges, events, nightlife spaces, online platforms).
· Coordinate in-person, digital, and social media–based outreach strategies.
· Ensure consistent presence in communities with high HIV incidence among YMSM.
· Monitor outreach coverage to ensure equitable geographic and demographic reach.
2. Enrollment & Linkage Oversight
· Monitor:
- Client enrollments
- Linkage to HIV testing
- Client enrollments
- Linkage to HIV testing
- PrEP referrals and initiation
- HIV-positive linkage to care (within 30 days)
- HIV-positive linkage to care (within 30 days)
· Ensure warm handoffs to Health Navigators and social service providers.
· Track follow-up to improve retention rates.
3. Data Management & Deliverables Monitoring
· Ensure all outreach contacts and client interactions are documented in required systems (AIRS, EHR, etc.).
· Review documentation for completeness and timeliness.
· Track monthly and quarterly performance metrics.
· Prepare reports for Program Manager and funders.
· Identify performance gaps and implement corrective action plans.
4. Community Partnerships & Stakeholder Engagement
· Build partnerships with:
- LGBTQ+ organizations
- Community-based organizations
- Schools and colleges
- Faith-based institutions (as culturally appropriate)
- Immigrant-serving organizations
- Represent the agency at community coalitions and planning meetings.
- Strengthen referral networks to support holistic client services.
5. Health Education & Prevention Programming
· Coordinate group workshops and community forums.
· Ensure evidence-based or culturally tailored prevention messaging.
· Promote:
- HIV testing
- PrEP and PEP education
- STI screening
- U=U messaging
· Oversee distribution of prevention materials and incentives per policy.
6. Compliance & Quality Improvement
· Participate in monthly/quarterly QI meetings.
· Assist with chart audits and outreach file reviews.
· Ensure confidentiality and HIPAA compliance.
· Support site monitoring preparation.
· Develop outreach improvement plans based on performance data.
QUALIFICATIONS
· Associate’s degree in Public Health, Social Work, Human Services, or related field preferred (experience may substitute).
· Minimum 1–2 years experience in HIV prevention, LGBTQ+ services, or community health.
· Demonstrated experience working with YMSM and/or LGBTQ+ communities.
· Knowledge of HIV prevention strategies, PrEP, and Ryan White services.
· Strong interpersonal and engagement skills.
· Ability to work evenings and weekends as needed.
· Minimum one (1) year of similar work experience, facilitating groups and performing Outreach/community engagement activities.
· Previous work experience with high risk, YMSM of color, and HIV+ populations.
· Excellent writing and communication skills; including multi-media outlets.
· Work experience with LGBTQ individuals in a medical/health or social service settings
· Knowledge of citywide resources and referral processes for LGBTQ individuals
· Experience in community based medical/health or social service settings, social work, counseling, nursing, public health or other health education.
· LGBTQ life experience
· Must pass background check

Compensation: $45,000.00 - $55,000.00 per year
